myPlace Health was founded in 2021 by mission-driven healthcare leaders and organizations (including SCAN Group and Commonwealth Care Alliance) that are committed to drastically improving health outcomes, quality and experience for vulnerable older adults and frail seniors. myPlace Health is an integrated care delivery organization that specializes in providing personalized care and coverage to older adults who wish to remain safely living in their homes and communities for as long as possible, rather than living in a nursing facility. Our mission is simple: to enable older adults to live the independent lives they deserve. The primary ways in which we pursue our mission are through capitated arrangements with government payers, such as Medicare and Medicaid under the PACE (Program of All-Inclusive Care for the Elderly) model, and as a value-based provider working in partnership with local health plans.

**Job Title** Project Manager – Healthcare (PACE Program) **Role Summary** Lead cross‑functional projects that enhance care delivery for frail older adults within a Program of All‑Inclusive Care for the Elderly (PACE). Drive initiatives from concept through completion, ensuring alignment with quality, compliance, and stakeholder expectations. Facilitate collaboration among clinical staff, operations, leadership, and external partners to achieve project objectives and continuous process improvement. **Expectations** - Manage end‑to‑end project lifecycle, deliver on time and within scope. - Build and sustain consensus among diverse stakeholders. - Maintain rigorous risk, quality, and regulatory oversight. - Communicate status clearly to all levels of the organization. - Adapt quickly to emerging priorities and changing environments. **Key Responsibilities** - Assemble and guide cross‑functional project teams (clinical, operations, leadership, external partners). - Define scope, objectives, budgets, and resource plans; track progress against milestones. - Identify and mitigate risks, developing contingency plans when necessary. - Ensure project deliverables meet organizational standards, regulatory requirements, and best practices in senior care. - Produce regular status reports, road‑maps, and post‑implementation reviews. - Champion process improvement initiatives, applying structured methodologies to streamline workflows. - Support change management activities, promoting adoption of new processes, tools, or systems. - Respond to ad‑hoc responsibilities and special projects as required to support organizational goals. **Required Skills** - Minimum 5 years of hands‑on project management experience, preferably in healthcare or related fields. - Proven record delivering cross‑functional projects from kickoff to completion. - Strong stakeholder‑management and influence skills; ability to build consensus. - Process‑improvement mindset; familiarity with Lean, Six Sigma, or similar tools. - Excellent organizational skills; able to juggle multiple projects, deadlines, and priorities. - Change leadership experience with successful adoption of new processes or technologies. - Proficiency in Microsoft Word, Excel, Outlook, and project management tools such as Smartsheet, Visio, or Microsoft Planner. - Strong communication and facilitation abilities. **Required Education & Certifications** - Bachelor’s degree in Business, Healthcare Administration, Public Health, or related field. - Project Management Professional (PMP) certification preferred; willingness to obtain within six months a ---
