About the Company

The North Bay Regional Health Centre (NBRHC) is a unique healthcare organization with three primary roles. It provides acute care services to North Bay and its surrounding communities, it is the district referral centre providing specialist services for smaller communities in the area, and it is the specialized mental health service provider serving all of northeast Ontario NBRHC has 389 beds and numerous outpatient and outreach services in North Bay and throughout the northeast region. NBRHC is one of four major acute care hospitals serving northeast Ontario; the others being Sault Area Hospital, Timmins and District Hospital and Health Sciences North (Sudbury). The area is also served by small community hospitals like Mattawa and West Nipissing General hospitals. NBRHC's Regional Mental Health Services provides inpatient beds in North Bay and Sudbury and outpatient and outreach services that throughout the region--from Hudson Bay to Muskoka from Sault Ste. Marie to the Quebec border. A major teaching centre for students in medicine, psychiatry, nursing and allied health professions, NBRHC is proud to be affiliated with the Northern Ontario School of Medicine, Nipissing University, Canadore College and several other Ontario colleges and universities.

Job Title: Nurse Practitioner – Geriatric Clinic Role Summary: Provide comprehensive geriatric care within a specialized outpatient clinic, developing collaborative treatment plans that enhance functional independence and well‑being of older adults. Expectations: Deliver evidence‑based assessment, diagnosis, and management of geriatric syndromes; prescribe safely within scope; coordinate multidisciplinary care; maintain high professional standards, documentation, and ethical practice. Key Responsibilities: - Perform comprehensive geriatric assessment (CGA, Barthel, CAM, MoCA, MMSE). - Diagnose and manage delirium, polypharmacy, falls, functional decline, acute/sub‑acute cognitive changes, bowel/bladder, nutrition/hydration. - Prescribe medication and interventions ethically and effectively. - Develop and implement team‑based treatment plans based on geriatric assessment principles. - Conduct therapeutic communication with patients and care partners, utilizing motivational interviewing and stages‑of‑change frameworks. - Coordinate care navigation and referrals across primary, community, and specialized geriatric services. - Participate in quality improvement, continuing education, and best‑practice implementation for geriatric and mental health care. - Manage multiple tasks, projects, and evolving priorities in a community outreach setting. - Travel as required within the community outreach team. Required Skills: - Advanced clinical competence in geriatric and geriatric mental health care. - Safe, ethical prescribing and medication management. - Proficiency with geriatric assessment tools and evidence‑based practice. - Strong therapeutic communication, documentation, and coordination skills. - Critical thinking, professional judgment, and problem‑solving. - Ability to handle physical demands and travel requirements. - Bilingual communication (English/French); French proficiency evaluated by formal test.
