**Job Title:** Clinical Coordinator, Wound Ostomy Continence
**Role Summary:**
Coordinate and deliver high‑quality, evidence‑based care for patients with complex wounds, ostomies, and skin conditions across inpatient and outpatient settings. Provide expert clinical assessments, individualized care plans, education, and discharge support, while driving quality improvement, research, and staff education initiatives within a multidisciplinary team.
**Expectations:**
- Deliver comprehensive, patient‑centered care using advanced wound, ostomy, and continence expertise.
- Lead and participate in quality improvement and research activities to enhance clinical outcomes.
- Educate patients, families, and healthcare staff on best practices and discharge planning.
- Collaborate effectively with interdisciplinary teams and departmental units.
- Maintain accurate, compliant documentation in electronic health records.
**Key Responsibilities:**
- Coordinate complex wound, ostomy, and skin care across care settings.
- Conduct expert assessments and provide consultations to develop individualized care plans.
- Create and deliver tailored educational materials for patients, families, and staff.
- Lead quality improvement initiatives; monitor outcomes and integrate evidence‑based practices.
- Contribute to academic and research projects; support workflow improvements.
- Maintain thorough clinical documentation in the electronic patient record system.
- Collaborate across departments and perform related duties within scope.
**Required Skills:**
- Advanced knowledge of wound, ostomy, and continence care.
- Strong clinical assessment and care‑plan development abilities.
- Proficiency in patient education and discharge planning.
- Experience with quality improvement and outcome evaluation.
- Effective teamwork, collaboration, and interdisciplinary communication.
- Excellent organizational and time‑management skills.
- Proficiency with electronic health records (EHR).
- Ability to teach one‑on‑one and in groups.
- Research and documentation skills (preferred).
**Required Education & Certifications:**
- Master’s degree in Nursing (preferred).
- Registered Nurse in good standing with the College of Nurses of Ontario.
- Basic Cardiac Life Support (B.C.L.S.) – Level II Rescuer.
- Advanced education/certification in wound, ostomy, and continence care (e.g., NSWOCC, WPC, WOCN, or WOCC(C)).
- Membership in the Registered Nurses Association of Ontario or related professional organization preferred.
- Minimum 5 years of relevant experience, including 2+ years in wound, ostomy, and continence specialty.
